How can this be done?

To save a tooth you need to put it back into the socket as soon as possible. The faster this is done the more chance you have of saving it. When you pick the tooth up pick it up by the crown not the root, it there is any debris on the tooth rinse it in milk or salin, put the tooth back into the socket, bite down onto it and contact your dentist.
